We have a full range of Care Services; many available on an in-patient and out-patient basis. Your Care Team will create a personalized treatment plan based entirely on where you are today, with a goal of helping you live a more independent, fuller life in the future. Many surgical procedures and health conditions require Short-Term Rehabilitation. We understand that you want to get back home and to your life as soon as possible, and that’s our goal, too.
Through short-term, intensive physical, and medical services, our rehabilitation team helps to restore each residents to his or her highest level of functioning and independence, and as quickly as possible. We provide Physical, Occupational, and Speech therapy six days a week based on your customized therapy treatment plan, which is determined by your diagnosis and treatment plan.

By adding a Physical Therapy regimen to your treatment plan, even the most painful injuries can heal faster and produce the best long-term results. Our physical therapists assist in restoring a wide range of impairments through therapeutic exercises and functional and walking training to enable our residents to reach their highest level of physical independence and mobility. Physical Therapy helps increase your strength and stamina through a series of exercises, examination, diagnosis and physical intervention.

Occupational Therapists focus on many daily living activities, such as bathing, dressing, grooming and safe transfers. By developing goals, we help our residents address the barriers that may hinder their performance and regain the skills necessary to leading a more rewarding life.
Our Occupational Therapists:
Loss of speech is frustrating, but can be regained partially or fully with the aid of trained speech therapists. Our certified and licensed Speech Therapy professionals are trained in the diagnosis and therapeutic techniques to improve speech reception, difficulty swallowing and other oral problems. We focus on cognitive retraining, expressive language and speech, swallowing skills training for any neurological/cognitive deficits, and other areas.

The goal of the Outpatient Therapy program is to give members of the community the option to live at home but still have access to the trained therapy staff and modern therapy gym. We currently offer Physical, Occupational, and Speech Therapy services on an outpatient basis.

Long-Term Care is offered at certain locations for any needs that our residents may have. We design individual care plans for each resident to address their needs and our compassionate caregivers provide 24-hour nursing care.

Our hospice care is designed to help our residents live out their last moments as comfortably as possible, and to allow family members to focus on saying goodbye.
We provide an intensive level of care that cannot be managed in other settings, including pain management as well as emotional and spiritual support.
Our residents receive highly compassionate and professional care management from a team of physicians, nurses, social workers, certified nursing assistants and chaplains. All in an environment that is calm, soothing and supportive of all.
Respite Care is a great opportunity when your caregiver needs a break. This beneficial service is offered for all service lines and includes dementia care. We will offer a fully furnished room for you to receive excellent care for up to 30 days.

When we were first notified about Covid-19, we immediately began preparing by establishing support units and collecting PPE. These units are designed for residents within our community who have tested positive. These units have designated staff and the added resource of a Respiratory Therapist to assist with respiratory complications. We are not admitting residents from the hospital with Covid-19, these units are for our internal residents and our residents returning from the hospital. Care provided in these units will be tailored to each resident and focused on reclaiming full health.
