We are committed to providing timely access to medical records. Please review the options below for submitting a medical records request and obtaining the information you need.
Printable Request Submission
To protect the privacy and security of our residents, all medical records requests must be submitted in writing. You may use our Printable Request Form to request a copy of your medical records or authorize the release of records to a designated third party. Please review the information below regarding who is eligible to request records. After completing the form, you may submit it directly to the facility in person, by U.S. mail, or via fax. Our team will process your request as promptly as possible.
Information Needed to Request Medical Records
If you are accessing health records through an existing Connected Care account, no additional verification is required. To access your records online, please visit the Connected Care Portal. If you do not currently have a Connected Care account and would like to establish one, please contact the facility’s Social Services Director for assistance.
Residents may request copies of their own medical records and will be required to present a valid form of identification. Individuals requesting records or authorizing the release of records on behalf of a resident must provide documentation demonstrating their legal authority to do so. Healthcare representatives, healthcare surrogates, healthcare powers of attorney, and legal guardians must present a valid form of identification and provide documentation verifying their legal authority to act on the resident’s behalf before records can be released.
